Plan Info will reply with confirmation of receipt. 

Review the form for completion and accuracy.

  • In Freshdesk search the name of the partner on the letter. There should be a ticket under the plan with their name in it.
    1. If there is not, let Rebecca Rice know
    2. Try searching NHC 401(k) and find the latest Indemnification letter ticket just in case we have a typo
  • Update the following in the ticket:
    1. Add a Note to the ticket with the following:
      • Notify to: Rebecca Rice and Michael Ranisate
      • Add the Name of the partner
      • Attach the letter by dragging the PDF to the note in Freshdesk
      • Also add: “In NHC folder” to the note, then add note
    2. If the letter is return mail:
      • Notify to: Rebecca Rice and Michael Ranisate
      • Add the Name of the partner
      • Add: “ID letter returned – bad address” to the note, then add note
